Uniform Mitigation Verification Inspection Form:

1. Citizens has not changed the Wind Mitigation rating charts in the manual to reflect the new form.
2. The new form has no level definitions and has a few new descriptions per section that they have not added to the charts.  Unfortunately you will need to read the definitions on the form and refer to the manual in the Wind MItigation section.
3. You may need to contact Citizens for those items that are not reflected in the charts such as: Roof Deck Attachment when Unknown, unidentified or no attic access is selected on the form by the inspector, or for Roof to Wall Attachment when Structural or Unknown is selected.
