 This checklist is found on Citizens website:  

•  Go to the Agent Resources menu on Citizens site and select Training and Reference Materials
•  Then click on the Checklists/Maps tab.
•  On the left side of the screen you will see a section called Agent New Business Submission Checklists.
•  Click on the Commercial Nonresidential Wind-Only (12/5/09) link.
