To obtain a forgotten user name and/or password:

1. If you forgot your Password:
   • 1)  After trying your login and it fails, you will be taken to a second user login screen.
   • 2)  This second user login screen will now have a link: Forgot Password? Click Here.
   • 3)  An Email Password Information  screen will come up and ask you to enter your User Name.
   • 4)  This password will be emailed to the main email address on your agency account.
2. If you forgot both your User Name and Password:
   • 1)  You will need to have the user with the Agency Administration access log in.
   • 2)  Select Users.
   • 3)  Click on Edit next to the user that forgot their login information.
   • 4)  This will display the Edit User Information screen where the User Name and Password  will be available.
3. If the Agency Administrator forgot their login:
   • 1)  Call Lori Cahill at 1-800-383-3482 ext 128.
   • 2)  You will be asked to verify your information to obtain the login.
