How do I access and print an ACORD Policy Change Request Form (Endorsement Form)?

Answer / Solution

ACORD Policy Change Request Form (Endorsement Form):

  1. Open your saved quote and rate it.
  2. Select the company you originally wrote the policy through.
  3. From the quote Breakdown screen, go to the Forms menu and select Policy Change Request. 
  4. The Agency Information including your Agency name and the Company name will prefill the fields as well as the Named Insured.
  5. You will need to make your selections based on the types of changes to the policy on this screen.
  6. Once you have completed the form, you can preview the form by clicking on the print preview icon or clicking on the Form menu at the top left of the screen and select Preview Form.
  7. Click on the print icon or click on the Form menu and select Print Form.
  8. Go to the Form menu and select Exit to close the screen.
